As vaccine requirements continue to rollout in San Francisco and throughout California, here's how you could keep a digital vaccine record on your phone:
- Access your digital vaccine record here.
- Input your first name, last name & your birthdate
- Input the phone number associated on your vaccine card
- Create a 4-digit PIN
Once the information is all matched within the system, you will be redirected to a new page with a QR code. When scanned, the QR code will pull up your existing COVID vaccine record. You may also screenshot this code for future use.
